Weekly Communicable Disease Report

No more faxes, telephone calls and emails coming from schools and daycare facilities for you to keep organized. Save time and effort working with secure, user friendly web based technology designed with you in mind.

KKzo LLC offers a Health Insurance Portability and Accountability Act (HIPAA) compliant web based application allowing you to accrue infectious disease statistics from schools and other sources for the purposes of reporting those statistics to the State of Michigan.

Entities using the reporting tool will be allowed to input data each reporting week from Monday 12:01 a.m. to Sunday 11:59 p.m. Subscribing counties will be provided access and tools to manipulate the data as needed to create reports for internal usage as well as for reporting to the state.

As part of the application, a secondary secure web location will be provided allowing counties to share files of selected information with other agencies. Control of these files, including addition and deletion of files, is granted to a single user responsible for the content. The same user will act as the administrative user for the county. Certain bandwidth limitations apply.

Security: (SSL)

  1. Data entry access accounts will be required (user and password) created and managed by a designated county employee. This same county employee will be the contact point for low-level technical assistance.

  2. Site users will enter data in an encrypted environment provided by KKzo.

  3. All administrative tools (such as report creating scripts) will operate exclusively in the secured environment.

  4. Each site user leaves a footprint resulting in an accountability log.

Reporting:

  1. Each county will have an administrative interface where they will be able to:
    • Add, Delete and Edit schools. This will include the login information for each school. The county will have the ability to disable school account access without deleting the account or data.
    • View the reports for each school and edit a schools report if errors have been found.
    • View the last date a school has reported and see an indication that the school made a section 2 report on the last report submitted.
    • View a totals report for sections 3, 4 and 5 of individual schools or all schools. This will show values by individual weeks. This report will be available in either HTML or CSV format and can be limited to a specified date range.
    • View a report for section 2. This report will show all section 2 reports for all schools. This report will be available in either HTML or CSV format and can be limited to a specified date range.
    • View a summary report for sections 3, 4 and 5. This report will show the total number of reported diseases for each school within the specified date range. This report will be available in either HTML or CSV format and can be limited to a specified date range.
    • View a report for the enabled schools who have not reported for a specific week. This report will be in HTML or CSV format.


  2. Each county will be assigned a specific HTTPS web address their schools will use to make reports. Users will be redirected to an HTTPS web address if they fail to type the address using HTTPS.

  3. Each school will have their own login allowing them to enter/add/edit the reporting information for the week. They will be able to continue updating the data until Sunday midnight. At this time, any new reporting will be for the following week.

  4. Each school will receive an automatic email notification to contact the county with weekly numbers any week they should fail to report.

Secure File Share Upload:

  1. Each county's authorized user will be able to upload files securely to the server. They will supply a password with each file. A URL will be provided that they will send to the person who is to download the file (probably via e-mail). They will then, using a secured means of communication, provide the password to the user.

  2. The user will then click on the provided URL. They will be prompted for their e-mail address and the password related to this file, and when the password has been validated, the file will download to the user.

  3. All file uploads and downloads will be logged to maintain high security. The user who uploaded the file, plus the date, time, file name, and description of the files will be logged. For downloads, the e-mail address and time of the download will be logged.

  4. The user at the county will then be able to delete the file from the server. When the file is deleted, the physical file will be deleted, but the related logs remain.

Support Services:

KKzo L.L.C. will provide your county representative with a thorough training session on the application and will provide a detailed FAQ for reference. Low level support will be provided to schools by the county representative trained on the application. Should an issue arise where advanced support is required, the county representative will contact KKzo via email requesting assistance. This provides the representative and KKzo with a record of the request and will assist with improvements to the application and FAQ. KKzo will respond in less than one business day in most cases.
